Make sure you have an itemized bill, your EOB, and relevant medical records. Identify specifically which charges you believe are incorrect and why.

How to dispute in writing

  1. Write a formal letter to the hospital billing department
  2. Identify the specific charges you're disputing and why each is incorrect
  3. Reference your itemized bill and EOB where relevant
  4. Request a written response within 30 days
  5. Send certified mail or email with read receipt
  6. Keep a copy of everything

If unresolved

Escalate to the hospital's patient advocate or patient services department. You can also file a complaint with your state insurance department for insurance-related disputes.
